Silvermorph Charity Ltd.於2023年4月開始成為社署「長者社區照顧服務券」(試驗計劃) Community Care Service Voucher (CCSV)的認可服務單位.。我們提供一系列家居為本 (Home-Based) 照顧和支援服務。
我們提供服務包括：
－個人照顧（扶抱、餵食、餵藥、洗澡、換片、簡單伸展運動、體位轉移等日常個人照顧。）
－ 復康訓練（根據物理/ 職業治療師所訂立的復康治療方案，由合資格復康助理執行。）
－ 護理服務（傷口護理 、腹膜透析 (洗肚)、注射糖尿針、造口護理 、管喂及專業護理等服務）
－ 環境評估及照顧者培訓（由合資格服務提供者根據長者的情況及生活習慣做評估及培訓，達致最佳的家居安老環境。）
－ 護送服務（安排人員陪同往返醫院覆診及匯報當日覆診情況。）
服務流程：
01 我們會約見申請人/ 家屬一次免費上門探訪，以便更瞭解個別的情況或需要。
02 當申請人/ 家屬決定選用我們，我們會收回「長者社區照顧服務券證明書」（“證明書”）並簽署相關文件。
03 根據我們收集的資料及判斷，我們會提供服務計劃安排 包括時間及服務提供者資料。護士亦會根據有關資料定制“個人護理計劃”。
04 我們會每月前一週左右寄出當月的服務計劃安排，以供參閱及調整。我們亦會定期跟進服務質素，以確保能提供足適合的服務內容。
費用 - 2023年4月 生效
長者須負責“社區券”上列明的百分比 (5% - 40%) 支付。政府會負責支付社區券服務組合價值的餘額。
專業人員 $909-945/小時
長者最低支付 $45/小時
支援人員 $238/小時
長者最低支付 $12/小時
例子1 - 每月40小時支援人員；百分比8%
總費用 = 40 * $238 = $9,280
$9,520 * 8% = $762 (長者負責)
例子2 - 每月11小時專業人員；百分比12%
總費用 = 11 * $945 = $10,395
$10,395 * 12% = $1,247 (長者負責)
例子3 - 每月40小時支援人員 +5小時專業人員；百分比12%
總費用 = 40 $238 + 5 $945 = $14,245
目前每月最多社區券面額為$10,260
=> 社區券部分: 12% * $10,260 = $1,231 (長者負責)
=> 其餘部分: 全費 (長者負責)
例子4 - 每月5小時支援人員 +1小時專業人員；百分比12%
總費用 = 5 $238 + 1 $945 = $2,135

Silvermorph Charity Ltd. had become a recognized service unit of the Social Welfare Department's "Community Care Service Voucher for the Elderly" (CCSV) in April 2023. We provide a range of home-based care and support services.
Our services include:
-Personal care (daily personal care such as transferring, feeding, giving medicine, bathing, changing diaper, simple stretching exercises, position transfer, etc.)
- Rehabilitation training (according to the rehabilitation treatment plan established by the physiotherapist/occupational therapist and carried out by qualified rehabilitation assistants.)
- Nursing services (wound care, peritoneal dialysis, diabetes injections, stoma care, tube feeding and professional nursing services, etc.)
- Environmental assessment and caregiver training (qualified service providers conduct assessment and training based on the elderly’s situation and living habits to achieve the best home environment for the elderly.)
- Escort service (arrange personnel to accompany patients to and from the hospital for follow-up consultations and report on the status of the follow-up consultations on that day).
Service Setting Process:
01 We will make an appointment with the applicant/family member for a free home visit to better understand the individual situation or needs.
02 When the applicant/family decides to choose us, we will take back the "Certificate of Community Care Service Voucher for the Elderly" ("Certificate") and sign the relevant documents.
03 Based on the information and judgment we collect, we will provide service plan arrangements including time and service provider information. The nurse will also customize a "personal care plan" based on relevant information.
04 We will send out the service plan arrangement for the month about one week before each month for reference and adjustment. We will also regularly follow up on service quality to ensure that we can provide adequate service content.
